WHAT IS MULTIPLE SCLEROSIS, ANYWAY?

Multiple sclerosis is an incurable autoimmune disease and is the second most common cause of disability in young adults. It typically affects adults aged 20 to 40 years. MS involves an inflammatory condition that damages myelin, thereby slowing down or completely halting the transmission of nerve impulses. Myelin becomes inflamed and detaches from nerve fibers, eventually transforming into hardened patches of scar tissue (sclerosis) that begin to form around the fibers. These formations appear in various areas of the brain and spinal cord. Due to the destruction of myelin, disturbances in the transmission of nerve impulses can occur, triggering numerous symptoms of multiple sclerosis. The cause of multiple sclerosis is not known. Possible factors influencing the development of multiple sclerosis include genetics, viral infections, and autoimmune disorders. Environment also plays a significant role, as factors such as heat, cold, stress, infections, vaccinations, and injuries can worsen clinical symptoms and signs.

COURSE OF THE DISEASE

The course of multiple sclerosis (MS) can vary widely. In 60-80% of patients, the disease begins with a relapsing-remitting (RR) form, where symptoms occur in episodes followed by partial or complete recovery. Episodes last more than 24 hours and recur at least 30 days apart without a fever. In 40-60% of RR patients, the disease transitions within 10-15 years to a secondary progressive (SP) form, characterized by a gradual worsening of symptoms with occasional relapses. Approximately 15% of patients have primary progressive (PP) MS, where symptoms continuously worsen without periods of improvement. This form more commonly affects older men and presents with symptoms such as lower limb weakness, increased muscle tone, coordination difficulties, spasms, speech or swallowing problems, and fatigue. There is also a benign form of MS, affecting 15-20% of patients, where symptoms do not significantly progress or progress at all even after 15 years from disease onset.
